Welcome to the RCN Children and Young People: Continuing and Community Care Forum community. This Forum supports members in delivering nursing care for children and young people with complex and continuing care needs that is seamless across all settings promoting integration of care through multi-professional and multiagency networks and inclusion in family life and society.

The RCN, in partnership with the Association of British Paediatric Nurses www.abpn.org.uk has produced this film to raise awareness surrounding the palliative care work undertaken by nurses in hospice and community settings.

The film follows a day in the life of Bernadette O'Gorman, a Children's Community Matron in London.
