Care plan for bone health and osteoporosis launched in Portsmouth

The Department of Rheumatology at Portsmouth Hospitals NHS Trust fully supports the Department of Health's implementation programme for individual personal care plans (IPCP) for people living with long term conditions.

To achieve this aim, Colin Beevor and Sr Amanda Finnegan have been working with Portsmouth and District Group of the National Osteoporosis Society and health care professionals to ensure appropriate information is available to patients at the time of diagnosis.

We have developed the IPCP for Bone Health and Osteoporosis that can be used by any health care professional to support the needs of people living with osteoporosis. It reflects the need for better and more-accessible information for people about bone health and Osteoporosis.

The IPCP was launched at the 2012 Love Your Bones Conference in Portsmouth on Wednesday 20 June 2012.
