Dear [Insert manager's name],
Pregnancy risk assessment
I am formally notifying you that I am [insert number of weeks] pregnant and require a pregnancy risk assessment to be carried out as a matter of urgency.
As my employer, under the Management of Health and Safety at Work Regulations 1999 (or if in Northern Ireland, the Management of Health and Safety at Work Regulations (Northern Ireland) 2000), you are legally required to carry out a pregnancy risk assessment.
As a pregnant member of staff, I may be more at risk of COVID-19. As my employer, you should be taking steps to assess my risk and put measures in place to reduce any harm to me or my unborn child.
The following guidance sets out what steps you need to take to protect me:
HSE, Protecting new and expectant mothers at work - www.hse.gov.uk/mothers/index.htm
I also request that this risk assessment is reviewed when I am approaching 28 weeks, so any necessary additional steps can be taken to protect me.
Please carry out this risk assessment as a matter of urgency.
If a risk assessment is not carried out within one week then I will be escalating this matter to the Royal College of Nursing.
I look forward to your response.
