The move to integrate health and care across England is changing the way health and social care is delivered. If you work as a member of nursing staff in England, it is important you are aware of how plans for delivering integrated care are progressing at local and regional levels.
NHS England’s Five Year Forward View made made clear that health and social care must be integrated, and over the past two years, ‘New Models of Care programme’, devolution plans, and more recently the creation of ‘Sustainability and Transformation Plans’ (STPs) are being used to achieve this aim.
