Our strategy and how we represent our forum members

Who we are and what we do

We are the RCN’s Children and Young People Continuing and Community Care Forum. This includes care such as- acute care in the community, long term conditions, complex care and palliative care. We support those working across a range of settings including; NHS Trusts, independent & charity sector, local authority and education. We regularly engage and represent the RCN at national meetings to ensure the Nursing voice helps to influence policy and practice changes. By bringing registered Nurses (including CYP & LD) and support workers who work in these services together with National stakeholders, we have developed guidelines that are used in everyday practice. We are also a place for those working or interested in these areas of work to seek support, network, learn and develop.

Forum committee

When fully recruited to, we have 7 Forum Committee Members including our Forum Chair Kate Medhurst. As committee members we have a wide range of specialist interests and are keen to add to this growing list.

Current areas of specialist interest include

Community care, Continuing care, Specialist School Nursing, Home Care (those working shifts in home settings), palliative and end of life care, pathways of care, oncology, management & leadership, workforce and community outreach care.

Specialist interest areas we’re looking to add to

Those working in clinical practice in; transitional care, specialist school nursing, community and continuing care, support worker roles, home care (working shifts in home settings), hospices, learning disabilities and those with an interest in developing data/digital outcomes. We are also looking for people working in a wide range of sectors including; commissioning, education, NHS commissioned services outside of NHS Trusts, charities and local authority.

The focus of our forum is continuing & community care, through our committee members and Specialist link roles we also help inform work that goes across the five CYP Forums to support a collaborative approach.
